SFCC: Third Attempt Enrollment Request
Introduction:  
Before you will be permitted to register for courses in excess of two attempts, you are required to complete this form.

The Policy: Per State Board for Community and Technical Colleges, students may only repeat a course twice after the initial enrollment for a total of three attempts.
Financial Aid Policy: Students must apply for and otherwise qualify to receive financial aid. Students who do not pass a course with a G.P.A. of 2.0 or better, may receive financial aid only once to repeat the course failed. Students who do successfully pass a course may be paid once to retake it.

Submitting this request does not guarantee a spot in the course, but we will try to add you when a seat becomes available, and you will not be officially enrolled until that happens.

Do not fill out the Repeat Success Contract section. This section will be filled out by the advisor who reviews your request.
