Introduction:  

Use this form to request reinstatement after an academic suspension. This form is for academic standing only and does not apply to financial aid appeals. If your financial aid status has changed, you will receive separate instructions from the Financial Aid Office.

Instructions:  

Appeals for Winter 2026 term must be submitted by December 29 at 11:59 PM. Late submissions will not be considered.
